Health |

plans available under CareFirst PPO HSA Option 2 (deductible
$2,000 individual; $4,000 family), CareFirst OA POS HSA (deductible
$1,200 individual; $2,400 family), or CareFirst OA HMO (no referral,
no deductible in-network).
- Prescription drugs require $0 copay for generic, $25 for preferred
brand name drugs, and $45 for non-preferred brand name drugs
for a 30-day supply. Monthly benefit allowance if you decline
medical coverage.

- Benefit of 1x annual earnings up to $250,000 max. Option to

Short Term Disability |

- Benefit = 60% of base weekly earnings to a maximum of $2,300.

- Flexible Spending Plan offers the opportunity to pay medical

- Immediate vesting of employer-paid contributions: 3%
