Flexible Spending Plan

  • Flexible Spending Plan offers you a choice to pay for certain medical and dependent care qualified benefits on a pre-tax basis. Maximum allowed - $2,400 for Medical (out-of-Pocket) expenses and $5,000 for Dependent Care Expenses.
 
 

Health

  • All monthly premiums covered. Coverage begins on first day of month following start date. Individual and family plans available under Aetna PPO HSA 1.3 plan. Deductible $1,200 Individual, $2,400 Family.
  • Prescription drugs require $10 copay for generic, $25 for preferred brand name drugs, and $50 for non-preferred brand name drugs for a 30-day supply.
  • Individual and family covered under Aetna HMO 1.3 plan. No deductible in-network, no coverage out-of-network. Prescription drug benefits identical to PPO HSA 1.3 Plan.
  • Monthly benefit allowance may be paid if you elect not to take medical coverage.
 
 

Dental

  • All monthly premiums covered. Individual and family plans available under MetLife PPO plan.
  • Benefits same for in- and out-of-network. Out-of-network visits may be subject to balance billing
  • Preventive procedures covered 100%. Basic procedures (fillings, simple extractions, etc.) covered 80%. Major procedures (crowns, dentures, etc.) covered 50%. Annual max $2,000. Orthodontic procedures covered 50%, $1,000 lifetime coverage.
 
 

Vision

  • All monthly premiums covered. Individual and family plans available under Avesis Advantage Vision Care plan. Vision exams require $10 copay. Standard lenses require $10 copay. $110 allowance given for contact lenses.
 
 

Base Life and AD&D

  • All monthly premiums covered. Group Life and AD&D coverage through UNUM. Benefit of 1x annual earnings up to $100,000 maximum.
 
 

Short Term Disability

  • All monthly premiums covered. Short Term Disability coverage through UNUM.
  • Benefit of 60% of base weekly earnings to a maximum of $1,500. Receive benefits on 1st day of accident, 8th day of sickness, and last up to 13 weeks.
 
 

Long Term Disability

  • All monthly premiums covered. Group Long Term Disability coverage with UNUM after Weekly Disability runs out.
  • Benefit of 60% of base monthly earnings up to $10,000 maximum.
  • Benefits begin after 90 days of disability, subject to pre-existing condition limitation.
 
 

Retirement

  • Employer paid contribution of 3% of monthly salary into 401K plan and 7% into profit-sharing plan. You can contribute up to maximum allowed by IRS.
 
 

EAP

  • Work-life balance employee assistance program provided at no cost to employee.
 
 

Paid Time Off (PTO)

Years of IT experience and commensurate PTO will be mutually agreed upon under the following guidelines:

  • PTO begins accruing first day of the month following employment. Annual accruals in eight-hour increments until 25 days max

Holidays

  • Receive all approved federal government holidays – considered ‘Floating Holidays’
  • Holidays taken in one-hour increments. Unused holidays are forfeited at end of year
 
 

Payroll

  • You are paid monthly on the 10th of each month.
 
 

Education/Training

  • Eligible consultants can receive up to $5,000 annually towards approved educational, training, and conference costs
